feburic对痛风有效吗？

Feburic, also known as febuxostat, was developed by Teijin Corporation of Japan. It is a new and highly effective non-purine xanthine oxidase selective inhibitor approved for marketing in the United States in 2009 for the treatment of hyperuricemia (gout). It was approved for listing in my country in 2018.

After a new drug is launched, the therapeutic effect is the most important for patients. So is feburic effective for gout?

The phase II clinical study of feburic (febuxostat) is a multi-center, double-blind, randomized trial that evaluates the safety and efficacy of feburic in gout. Gout patients included in the trial, including 17 women and 136 men, were randomized to receive feburic (40, 80 or 120 mg/day) or placebo.

The test results after 4 weeks found that the serum uric acid concentration of patients treated with placebo was only reduced by 2%, while the serum uric acid concentration of patients in each dose group of feburic was significantly reduced compared with before treatment, with an average reduction of 37%, 44% and 59% in each group from low to high dose respectively. The effect was significant.

For 30 years, allopurinol has been the only drug clinically used to inhibit uric acid production. Compared with allopurine, feburic also has very obvious advantages. Unlike allopurinol, which only inhibits the reduced form of XOR, feburic (febuxostat) also has a significant inhibitory effect on the oxidized form of XOR, so its uric acid-lowering effect is more powerful and lasting.

During allopurinol treatment, in order to reduce the effects of purine and pyridine metabolism on the activities of other enzymes, repeated high-dose administration is required to maintain high drug levels. This also leads to serious or even fatal adverse reactions due to drug accumulation. Feburic (febuxostat) is a non-purine XOR inhibitor, so its safety is also higher.
