Dosage when pemetinib is used in combination with CYP3A inhibitors (itraconazole, voriconazole, ritonavir)

Avoid coadministration of strong and moderate CYP3A inhibitors with pemetinib. If coadministration with strong or moderate CYP3A inhibitors cannot be avoided, a dose reduction of pemetinib may be required when coadministered with CYP3A inhibitors (itraconazole, voriconazole, ritonavir):

Reduce the dose from 13.5 mg to 9 mg.

Decrease the dose of pemetinib from 9 mg to 4.5 mg.

If a concomitant drug that is a strong or moderate CYP3A inhibitor is discontinued, increase the pemetinib dose (after 3 plasma half-lives of the CYP3A inhibitor) to the dose before starting the strong or moderate inhibitor.

When treating cholangiocarcinoma, the recommended dose of pemetinib is 13.5 mg, taken orally once daily for 14 days, followed by 7 days off, for a total cycle of 21 days. Continue treatment until disease progression or unacceptable toxicity.

The recommended dose of pemetinib for the treatment of FGFR1-rearranged myeloid/lymphoid neoplasms is 13.5 mg orally once daily. Continue treatment until disease progression or unacceptable toxicity.

Take pemetinib at approximately the same time each day (with or without food) and swallow the tablet whole. Do not crush, chew, split or dissolve tablets. If the patient does not take pemetinib on time for 4 hours or more, or vomits, continue taking the drug at the next planned dose.

Pemetinib is a kinase inhibitor drug that can block the proliferation of cancer cells and slow down the spread of cancer cells. It can be used to treat adult patients with previously treated, unresectable locally advanced or metastatic cholangiocarcinoma and adult patients with relapsed or refractory myeloid/lymphoid neoplasms (MLN) associated with fibroblast growth factor receptor 1 (FGFR1) rearrangement.

Use with caution in those allergic to pemetinib. Many other drugs may also interact with pemetinib, and your doctor should be informed of the drugs you are using, especially St. John's wort. Do not eat large amounts of grapefruit or drink grapefruit juice while taking this medicine. It is recommended that patients take medication under the guidance of a doctor.
