It is a first-in-class anti-epileptic drug (AED) that is a highly selective, non-competitive AMPA-type glutamate receptor antagonist. Glutamate is the primary neurotransmitter mediating epileptic seizures. As an AMPA receptor antagonist, parampanel can reduce the over-excitability of nerves associated with epileptic seizures by targeting the activity of postsynaptic AMPA receptor-glutamate; this mechanism of action is different from currently commercially available anti-epileptic drugs (AEDs).

Paraampanel is indicated for the add-on treatment of patients with partial-onset epilepsy (with or without secondary generalized seizures) in adults and children 12 years of age and older.

The effect of parampanel in treating epilepsy:

In the treatment of partial-onset seizures, garampanel has been compared with placebo (dummy treatment) in three major studies, involving a total of 1,491 patients aged 12 years and older with partial-onset seizures that had not responded to other treatments. In these studies, valampanel was administered at doses of 2, 4, 8, or 12 mg daily for up to 19 weeks. All patients also took other antiepileptic drugs. The primary measure of effectiveness was the percentage of patients who experienced at least a 50% reduction in seizure frequency.

For primary generalized tonic-clonic seizures, valampanel was shown to be effective in a study involving 164 patients with idiopathic generalized epilepsy. Parampanel was compared with placebo as an addition to patients' existing epilepsy treatments. Initiate treatment at a dose of 2 mg of garampanel and, if tolerated, gradually increase to a maximum of 8 mg over 4 weeks and then continue for a further 13 weeks. The primary measure of effectiveness was the percentage of patients who experienced at least a 50% reduction in seizure frequency.

Most patients have only mild side effects after taking perampanel. Common ones include dizziness, drowsiness, headache, fatigue, irritability, nausea and falls, and most are mild to moderate. If side effects occur, you need to contact your doctor. Side effects will be reduced when the dosage titration is slowed down and the dosage of concomitant medications is adjusted. Patients should remember to stop taking medication or adjust dosage without permission.
