  1.Position and Eligibility

  Stiripentol(common brand Diacomit)is an oral antiseizure agent for Dravet-syndrome–related seizures as adjunctive therapy.All three criteria should be met:age 6 months or older,body weight 7kg or more,and concurrent clobazam therapy with confirmed Dravet syndrome.Monotherapy is not supported by the approved label,so stopping clobazam or using stiripentol alone is not standard.If clobazam is not used,age is under 6 months,weight is below 7kg,or the seizure type is not clearly Dravet-related,complete genetic testing,EEG,seizure classification,and combination review before initiating.Selected other severe myoclonic epilepsies may be discussed off-label,but age,hepatic/renal status,and evidence must be weighed by a specialist.

  2.Dosing and Administration

  Recommended total dose is 50mg/kg/day by mouth in 2 or 3 divided doses:

  6 months to under 1 year,7kg or more:generally 25mg/kg twice daily to limit fluid load;

  1 year and older,7 to under 10kg:25mg/kg twice daily;

  1 year and older,10kg or more:25mg/kg twice daily or 16.67mg/kg three times daily;

  Round to the nearest feasible dose within about 50–150mg of the 50mg/kg/day target using 250mg and/or 500mg strengths;

  Maximum total dose 3000mg/day.

  Swallow capsules whole with a glass of water during a meal;do not break or open.For oral suspension powder,mix in about 100mL water,take immediately with food,and rinse the cup with a small amount of water to take residual drug.If a dose is missed,take it when remembered unless the next dose is due;never double.Taper rather than stop abruptly to reduce seizure-frequency rebound or status epilepticus;rapid discontinuation,when medically necessary,requires monitoring.When adding stiripentol to clobazam,valproate,or multiple antiseizure drugs,specialists may up-titrate over several days and delay target dose if somnolence,poor feeding,vomiting,or weight loss appears.

  3.Safety and Laboratory Monitoring

  Common adverse reactions occurring in at least 10%and more than placebo include somnolence,decreased appetite,agitation,ataxia,decreased weight,hypotonia,nausea,tremor,dysarthria,and insomnia.In controlled Dravet studies with clobazam,somnolence was about 67%with stiripentol versus 23%with placebo,so excess sedation should be attributed to the combination,not stiripentol alone.

  Monitoring priorities:

  Sedation/CNS depression:avoid alcohol and unnecessary sedating antiseizure drugs,opioids,or benzodiazepines;counsel against driving or hazardous machinery while alertness is uncertain.If somnolence appears with clobazam,consider reducing clobazam by 25%initially and reassessing further reductions or other sedating drugs if it persists.

  Appetite and growth:track weight,height,and caloric intake in children;if valproate is co-used and appetite/weight fall markedly,a neurology-led valproate reduction of about 30%per week may be discussed.

  Blood counts:baseline CBC before start and approximately every 6 months;investigate neutropenia or thrombocytopenia with infection/bleeding context before continuing,holding,or reducing.

  Hepatic/renal:avoid or use caution in moderate–severe hepatic or renal impairment;check LFTs and metabolic parameters when multiple drugs are used.

  Mood/suicide risk:all antiseizure drugs can increase depressive or suicidal thinking;caregivers should report mood or behavior changes.

  PKU:suspension contains phenylalanine(about 1.40mg per 250mg packet,2.80mg per 500mg packet);capsules do not.Choose formulation by total daily phenylalanine budget.

  Withdrawal:taper over weeks to months based on seizure frequency,combination regimen,and pregnancy planning.

  4.Interactions and Product Selection

  Stiripentol inhibits metabolic pathways involving CYP1A2,CYP2C19,and CYP3A4 and can affect CYP2B6,CYP2C8,P-gp,and BCRP substrates:

  Clobazam and its active metabolite levels rise;overt sedation,ataxia,or agitation should trigger clobazam review before arbitrary stiripentol changes.

  Review diazepam,clopidogrel,methotrexate,prazosin,glyburide,theophylline,caffeine,sertraline,midazolam,and similar drugs for needed dose adjustment.

  Strong inducers such as rifampin,phenytoin,phenobarbital,or carbamazepine may lower stiripentol exposure;avoid or re-titrate.

  Other CNS depressants and alcohol increase somnolence,especially in children.

  Different markets offer originator and compliant generic stiripentol;capsules are commonly 250mg and 500mg,and suspension powder comes in 250mg or 500mg packets.Choose pack size by daily weight-based pill count,ability to combine 250/500mg strengths,child’s ability to swallow capsules versus needing suspension,travel/storage conditions,batch number,expiry,marketing authorization,and manufacturer quality documentation—not by“big box is cheaper.”Do not use unapproved,unbatched,unlabeled,or improperly stored products;cross-border purchasing carries legal,authenticity,and storage risks,so all switches should go through the prescribing hospital and pharmacist.
