Modern medicine believes that epilepsy (IS) belongs to the category of neurodegenerative diseases, and its mechanism is related to the imbalance of the excitatory transmitter glutamate (glutamicacid) and the inhibitory transmitter γ-aminobutyric acid (GABA) in the brain, and can also cause dopamine, 5-hydroxytryptamine

The imbalance of monoamine neurotransmitters, including the neurotransmitter, causes structural and functional changes in brain areas related to the regulation of mood and cognitive functions. Antiepileptic drugs are structural analogs of GABA and are mainly used to treat partial seizures of epilepsy, infantile spasms, or as auxiliary drugs for secondary generalized seizures. The role of Vigabatrin

As a structural analog of GABA, the main mechanism of action of Vigabatrin is to competitively antagonize GABA, irreversibly inhibit GABA-T to delay the metabolism of GABA, increase GABAergic neurotransmission and its mediated neuroinhibition, and achieve the effect of inhibiting epileptic seizures. Studies have shown that Vigabatrin can also inhibit the uptake of GABA by glia and stimulate its release through the blood-brain barrier, and reduce the circulation of glutamate or glutamine between nerves and cells, thereby inhibiting epileptic seizures.

Effects of Vigabatrin

Exploratory studies have shown that treatment with Vigabatrin stopped attacks in 38% of IS patients. Among them, IS caused by tuberous sclerosis responded best to vigabatrin (response rate 87.5%). Symptomatic IS patients with known causes had better curative effects than cryptogenic patients. Subsequent randomized controlled studies have shown that Vigabatrin 150 mg·kg·d-1 monotherapy terminated all attacks in 11 patients with tuberous sclerosis IS, and its efficacy was significantly better than hydrocortisone as a control. Patients who were ineffective or intolerant to hormone therapy also had their attacks under control after being transferred to Vigabatrin.

Vigabatrin side effects

Found in multiple studies:

1. Common symptoms include fatigue, drowsiness, headache, dizziness, confusion and depression.

2. Occasional aggressive behavior and psychosis (more common in patients with a history of mental illness and behavioral problems), memory loss, diplopia, weight gain, gastrointestinal disorders, edema and hair loss.

3. Nystagmus, nervousness, irritability, ataxia, paresthesia and tremor may also occur.
