Is rufinamide useful for tonic seizures?

Useful. (Rufinamide) is a new type of anti-epileptic drug that exerts anti-epileptic effects by regulating sodium channel activity. It is mainly used for the adjuvant treatment of Lennox-Gastaut syndrome (LGS). In addition, rufinamide is also effective for some refractory epilepsy syndromes in children and adults. Recently, British scholars have used rufinamide as an auxiliary drug to treat refractory tonic status epilepticus.

Rufinamide in the treatment of refractory tonic epilepsy

A multicenter randomized placebo-controlled parallel trial (n=50) showed that rufinamide as an adjuvant treatment for patients with partial seizures or early generalized tonic-clonic seizures can reduce the patients' seizure frequency by an average of 41% from baseline, while the average increase in the placebo group was 52% (P=0.040).

In a multicenter, double-blind, randomized, placebo-controlled, parallel-group study evaluating the efficacy of rufinamide as an adjuvant treatment for LGS-related seizures, 138 patients received rufinamide or placebo for 12 weeks after completing a 4-week stabilization phase.

The results showed that the average frequency of epileptic seizures in the treatment group was reduced by 32.7% every 28 days, while the average reduction in the placebo group was only 11.7% (P=0.0015). In addition, the proportion of patients with improved seizure severity in the treatment group, 53.4%, was higher than that in the placebo group, 30.6% (P=0.0041).

Rufinamide side effects

1. The main symptoms of rufinamide in the central nervous system include drowsiness, dizziness, headache, fatigue, gait disorder, and ataxia. 2. Other adverse reactions include nausea, vomiting, decreased or increased appetite, upper abdominal pain, constipation, indigestion, rash, nasopharyngitis, diplopia, anemia, frequent urination, tremor, back pain, etc.

3. The main adverse reactions leading to drug withdrawal are convulsions, drug rash, fatigue, dizziness, etc.
