The efficacy and role of Vigabatrin
Vigabatrin, or vigabatrin, is an anti-epileptic drug, but it only reduces seizures and does not eliminate or cure the underlying epilepsy. Vigabatrin is a selective, irreversible inhibitor of the enzyme-activated GABA transaminase that acts by inhibiting GABA-T and thereby increasing levels of the inhibitory neurotransmitter GABA. Vigabatrin is currently approved for use in combination with other medications to treat complex partial seizures in adults and children at least 10 years old and for infantile spasms in infants and children 1 month to 2 years old.
Complex partial seizures (CPS) are a type of focally related seizures that carry serious complications and risks. CPS is difficult to treat and may become resistant to antiepileptic drug (AED) treatment. Refractory CPS (rCPS) is extremely dangerous as it can cause severe functional impairment and physical harm, sometimes with fatal consequences. Control of seizure activity is critical to the clinical treatment of CPS.
is a unique AED approved in Europe and the United States for the adjunctive treatment of adult patients with rCPS who have responded poorly to several alternative therapies. This review focuses on appropriately controlled studies of Vigabatrin conducted in Europe.
Several double-blind studies randomized rCPS patients to receive vigabatrin versus placebo, and two of these studies evaluated the durability of long-term, open-label treatment responses with vigabatrin. Study endpoints included seizure frequency, treatment satisfaction, and adverse events (AEs).
Efficacy results
Vigabatrin add-on therapy significantly reduces seizure frequency. Long-term studies have shown that responses to Vigabatrin therapy are durable and tolerable for years. Treatment satisfaction data showed that physicians and study participants were more likely to use Vigabatrin compared with placebo. Vigabatrin is well tolerated, and AEs common to AEDs are generally mild. Effects on vision were not formally monitored in these studies. In European trials, Vigabatrin was effective as an adjuvant therapy for rCPS.
Vigabatrin side effects
Symptoms of Vigabatrin include fatigue, drowsiness, headache, dizziness, confusion, and depression. Nystagmus, nervousness, agitation, ataxia, paresthesia, and tremor may also occur. Occasional aggressive behavior and psychosis (more common in patients with a history of psychosis and behavioral problems), memory loss, diplopia, weight gain, gastrointestinal disturbances, edema, and alopecia. Children may become excited and anxious.
Treatment of side effects of Vigabatrin
1. If these symptoms occur, it is recommended to rest for enough time to avoid overexertion. If symptoms are severe or persist for a long time, talk to your doctor, who may recommend adjusting your medication dose or giving you other treatments.
2. If you experience confusion and depression, nystagmus, nervousness, irritability, ataxia, paresthesia and tremor, aggressive behavior and psychosis, please be sure to contact your doctor.
