The recommended starting dose of tolvaptan is 15 mg, once a day, and can be taken before or after meals. However, patients should note that changes in serum electrolytes and blood volume should be frequently detected when taking the drug for the first time and when increasing the dose. Patients should avoid restricting fluid intake during the first 24 hours of treatment and should drink water when thirsty.
