(Sirolimus), suitable for patients receiving kidney transplants to prevent transplant organ rejection. Clinical recommendations include the use of Rapamune preparations in combination with cyclosporine and corticosteroids.
Rapamin (sirolimus) usage and dosage: It is recommended to use it in combination with cyclosporine and corticosteroids. It needs to be taken orally, once a day. It should be started as early as possible after transplantation. For new transplant recipients, the loading dose should be taken, which is 3 times the maintenance dose. The recommended loading dose for kidney transplant patients is 6 mg and the maintenance dose is 2 mg/day. Although in clinical trials, a loading dose of 15 mg and a maintenance dose of 5 mg/day were used, the therapeutic benefit of doses above 2 mg is unclear for kidney transplant patients.
Things to note when using Rapamin (sirolimus):
1. Grapefruit juice can slow down the metabolism of this drug regulated by CYP3A4, so it cannot be used to take or dilute this drug. It is recommended to take rapamycin 4 hours after taking cyclosporine oral solution (improved version) and/or cyclosporine capsules (improved version).
2. Patients taking Rapamin (sirolimus) should be given complete dosage instructions. Women who may become pregnant should be informed of the possible risks during pregnancy, and should take effective contraceptive measures before starting treatment with Rapamin (sirolimus), during treatment and within 12 weeks after stopping treatment.
Warning:
Immunosuppression may increase susceptibility to infections and may lead to the development of lymphoma and other malignancies, especially skin cancer. Excessive suppression of the immune system also increases susceptibility to opportunistic infections, sepsis, and fatal infections. Rapamin (sirolimus) should only be used by physicians experienced with immunosuppressive therapy and treating kidney transplant patients. Patients receiving this drug should be treated in an institution with appropriate laboratory and ancillary medical facilities and personnel. Physicians responsible for maintenance therapy should have complete information necessary for patient follow-up.
Patients receiving rapamycin (sirolimus) were more likely to experience increases in serum cholesterol and triglycerides requiring treatment than patients receiving azathioprine or placebo.
In a phase III trial, patients who received rapamycin (sirolimus) plus cyclosporine had an increase in mean serum creatinine and a decrease in mean glomerular filtration rate compared with a control group of patients who received cyclosporine plus placebo or cyclosporine plus azathioprine. Renal function should be monitored during maintenance regimens that include immunosuppression concomitantly administered with cyclosporine and rapamycin (sirolimus); appropriate adjustments to the immunosuppressive regimen should be considered when patients' serum creatinine values rise. Caution should be used when using drugs known to impair renal function.
