Febuxostat (febuxostat, feburic) is a new generation of xanthine oxidase inhibitor developed by Teijin Corporation of Japan. It is clinically used to treat hyperuricemia (gout). Purine nucleotides in food are mainly oxidized to uric acid in two steps by xanthine oxidase. Febuxostat is a new and highly efficient non-purine xanthine oxidase selective inhibitor that can directly inhibit the activity of xanthine oxidase and reduce the production of uric acid.

In 2009, febuxostat was approved for marketing in the United States. Its main ingredient is febuxostat, tablets. From the development and use of the first acid-lowering drug for gout to today, treatment options for gout patients have been continuously developed and increased. Febuxostat is a safe and effective drug for the treatment of gout, and diet does not affect its acid-lowering effect.

What should I pay attention to when taking Febuxostat?

1. Gout attack. An increase in gout attacks was observed after initiation of treatment with febuxostat. This is caused by changing serum uric acid levels that reduce the activity of deposited urate. To prevent gout attacks while administering this product, concomitant administration of a nonsteroidal anti-inflammatory drug or colchicine is recommended.

2. Cardiovascular diseases. In randomized controlled studies, patients taking febuxostat were more likely to have cardiovascular thrombotic events (cardiovascular death, nonfatal myocardial infarction, nonfatal stroke) than patients taking allopurinol. The reasons have not yet been clarified. Monitor for signs and symptoms of myocardial infarction (MI) and stroke.

3. Increased liver enzymes. In randomized controlled studies, transaminase levels were observed to be higher than 3 times the upper limit of normal (AST: 2%, 2%, ALT: 3%, 2% in patients treated with this product and allopurinol, respectively). No dose-response relationship was found for the increase in transaminases. Liver function laboratory analysis recommends treatment with febuxostat for 2 and 4 months, followed by periodic treatment thereafter.
