Febuxostat (febuxostat, feburic) is a new generation of xanthine oxidase inhibitor developed by Teijin Corporation of Japan. It is clinically used to treat hyperuricemia (gout). Purine nucleotides in food are mainly oxidized to uric acid in two steps by xanthine oxidase. Febuxostat is a new and highly efficient non-purine xanthine oxidase selective inhibitor that can directly inhibit the activity of xanthine oxidase and reduce the production of uric acid.

The Phase II clinical study was a multicenter, double-blind, randomized trial that evaluated the safety and efficacy of febuxostat (febuxostat, feburic) in gout. Gout patients included in the trial, including 17 women and 136 men, were randomized to receive febuxostat (40, 80 or 120 mg/day) or placebo.

The test results after 4 weeks found that the serum uric acid concentration of patients treated with placebo was only reduced by 2%, while the serum uric acid concentration of patients in each dose group of febuxostat was significantly reduced compared with before treatment, with an average reduction of 37%, 44% and 59% in each group from low to high dose, respectively. The effect was significant.

For 30 years, allopurinol has been the only drug clinically used to inhibit uric acid production. Compared with allopurine, feburic also has very obvious advantages. Unlike allopurinol, which only inhibits the reduced form of XOR, febuxostat (febuxostat, feburic) also has a significant inhibitory effect on the oxidized form of XOR, so its uric acid-lowering effect is more powerful and lasting.

The initial recommended dose of febuxostat (febuxostat, feburic) is 40 mg/day, administered orally. If the symptoms are severe, it can be adjusted to 80 mg/day; if the serum uric acid level is still higher than 6 mg/dL after taking 40 mg/day for 2 weeks, the dose can be increased to 80 mg/day. Febuxostat is not recommended for the treatment of asymptomatic hyperuricemia.
