Perampanel is a non-competitive α-amino-3-hydroxy-5-methyl-4-isoxazole receptor (AMPAR) antagonist with anti-epileptic effects and is mainly used clinically to treat epilepsy. For the treatment of partial-onset epilepsy in patients ≥4 years of age, with or without secondary generalized seizures. For the adjuvant treatment of patients with primary generalized tonic-clonic epilepsy aged ≥12 years.

Study 304, Study 305 and Study 306 were all rated as Level I evidence by the 2018 AAN/AES Guidelines. The designs of Study 304, Study 305 and Study 306 are basically similar, and they are all multi-center, randomized, double-blind, placebo-controlled studies. Three studies conducted at different research centers showed little difference in the 50% effective rate and seizure-free rate of perampanel at the same dose.

Among them, the 50% effective rates of perampanel 8mg combination therapy in studies 304, 305 and 306 were 37.6%, 33.3% and 34.9% respectively, and the seizure-free rates were 2.6% and 2. 8% and 4.8%; in Study 304 and Study 305, the 50% effective rates of perampanel 12 mg combination therapy were 36.1% and 33.9%, respectively, and the seizure-free rates were 2.0% and 6.5%, respectively.

In Study 306, the effective rates of perampanel 2mg and 4mg combined treatment were 20.6% and 28.5% respectively, and the seizure-free rates were 1.9% and 4.4% respectively. These data may not seem high, but special attention should be paid to the fact that these treatments are for "refractory epilepsy", and such a "50% effective rate" and "seizure-free rate" are very good performances.
