It is suitable for the additional treatment of adults and children aged 12 years and above with partial seizures of epilepsy (with or without secondary generalized seizures).

How to take Parampanel:

1. The recommended starting dose of perampanel is 2 mg orally once a day before bedtime. Increase dose by 2 mg increments daily infrequently weekly to a dose of 4 mg to 8 mg once daily at bedtime. In elderly patients, it is recommended that the dose be increased less frequently than every 2 weeks during the titration period.

2. The recommended dosage range is 8 mg to 12 mg once a day. A dose of 12 mg once daily resulted in a somewhat greater reduction in seizure rates than a dose of 8 mg once daily, but with a significant increase in adverse effects. Individual dosing should be adjusted based on clinical response and tolerability.

3. Presence of enzyme-inducing anti-epileptic drugs

In the presence of enzyme-inducing antiepileptic drugs, including phenytoin, carbamazepine, and oxcarbazepine, the recommended starting dose of perampanel is 4 mg and patient response should be monitored closely. Clinical trials revealed significant reductions in seizure rates in these patients. The reduction in seizure frequency was somewhat greater at 12 mg than at 8 mg.

when these enzyme-inducing antiepileptic drugs are introduced or withdrawn from a patient's treatment regimen. Patients should be closely monitored for clinical response and tolerability. Perampanel dose adjustment may be necessary.

4. Dosage adjustment

Dose adjustment in patients with hepatic impairment:

Dose adjustment is recommended based on higher exposure and longer half-life of perampanel in patients with mild and moderate hepatic impairment. The starting dose should be 2 mg daily with weekly increments of 2 mg daily every 2 weeks until the target dose is achieved. The maximum recommended daily dose is 6 mg for patients with mild hepatic impairment and 4 mg for patients with moderate hepatic impairment. Dosage increases in patients with mild to moderate hepatic impairment, as in all patients, should be based on clinical response and tolerability. No use is recommended in patients with severe hepatic impairment.

5. Patients with renal impairment

Perampanel can be used with close monitoring in patients with moderate renal impairment. Slow titration may be considered based on clinical response and tolerability. Not recommended for use in patients with severe renal impairment or in patients undergoing hemodialysis.
