Which drugs cannot be used with vigabatrin oral solution powder?

1. Anti-epileptic drugs:

Phenytoin

Although routine phenytoin dose adjustments are not required, phenytoin dose adjustments should be considered if clinically indicated because administration of vigabatrin oral solution may result in modest reductions in total phenytoin plasma levels.

Clonazepam

Powdered vigabatrin oral solution may moderately increase clonazepam, resulting in an increase in clonazepam-related adverse reactions.

Other antiepileptic drugs: There are no clinically significant pharmacokinetic interactions between vigabatrin and phenobarbital or sodium valproate. Based on population pharmacokinetics, carbamazepine, cloracide, primidone, and sodium valproate appear to have no effect on plasma concentrations of vigabatrin.

2. Oral contraceptives

Vigabatrin is unlikely to affect the efficacy of steroid oral contraceptives.

Three Drug-Lab Test Interactions

May reduce alanine aminotransferase (ALT) and aspartate aminotransferase (AST) plasma activities in up to 90% of patients. In some patients, these enzymes become undetectable. Inhibition of ALT and AST activity by vigabatrin may hinder the use of these markers, especially ALT, to detect early liver injury.

Vigabatrin may increase the levels of amino acids in the urine, possibly causing false-positive tests for some rare inherited metabolic diseases, such as alpha-aminoadipic aciduria.

The effects of vigabatrin on breastfed infants and milk production are unknown. Because of the potential for serious adverse effects of vigabatrin in nursing infants, breastfeeding is not recommended. If a breastfed infant is exposed to vigabatrin, observe for any potential risk.

The safety and efficacy of vigabatrin as adjunctive therapy for refractory complex partial seizures in children younger than 2 years of age and as monotherapy for the treatment of infantile spasms in pediatric patients younger than 1 month of age have not been established. Abnormal MRI signal changes and intramyelin edema (IME) have been observed in infants and young children receiving vigabatrin.
