非奈利酮正确用法用量是怎样的?需要注意什么?
The third-generation potent, highly selective, non-steroidal mineralocorticoid receptor antagonist (MRA) fenelidone was officially approved by the US FDA for "diabetic nephropathy" on July 9, 2021. It can reduce the risk of functional decline, non-fatal heart attack, renal failure, cardiovascular death, and heart failure hospitalization in adults with type 2 diabetes and chronic kidney disease.
So, what is the correct dosage of fenelinone?
The dosage of fenelidone will be adjusted according to the patient's different conditions. Based on the estimated glomerular filtration rate (eGFR) and serum potassium threshold, the recommended starting dose is 10 mg or 20 mg, taken orally once a day. Fenelidone tablets can be taken on an empty stomach or with food. Based on eGFR and serum potassium thresholds, increase dose to target dose of 20 mg once daily after 4 weeks. Patients must follow the doctor's instructions and do not change the dosage and usage at will to avoid intolerance reactions. In patients with chronic heart failure, type 2 diabetes, and/or chronic kidney disease with reduced ejection fraction, fenelinone and eplerenone have the same effect and can significantly reduce NT-proBNP levels, but they can also moderately reduce patient mortality and cardiovascular hospitalization rates.
So, what should you pay attention to during fenelidone treatment?
Fenelidone is a non-steroidal mineralocorticoid receptor antagonist (MRA). Hyperkalemia, hypotension and hyponatremia are its common adverse reactions. Patients should note that it is contraindicated in patients with adrenal insufficiency during the medication. It is prohibited to use strong CYP3A4 inhibitors at the same time. Monitor serum potassium levels during treatment and adjust the dose as needed. Avoid simultaneous use with grapefruit or grapefruit juice. Breastfeeding is not recommended when taking the medication. Patients can read the drug package insert for more details.
