Valadostat is an oral HIF-PH inhibitor used to treat anemia in patients with dialysis-dependent chronic kidney disease. It improves anemia symptoms by regulating erythropoietin and iron metabolism. It was approved in Japan in 2020 and by the US FDA in 2023.

What are the ways to purchase genuine Vafseo?

Vadostat is currently not included in China’s medical insurance and has not been approved for marketing. Patients need to purchase it through the following two formal channels.

Purchase overseas

Patients can go to medical institutions or pharmacies in listed countries and purchase it with a doctor's prescription. The original drug produced by MEDICE in Germany has a specification of 300mg*98 tablets/box, and the price is about US$320.

The advantage is that drug qualifications can be directly verified and the risk of counterfeit drugs is reduced. The disadvantage is that international travel costs and time need to be borne, and local hospitals need to be contacted in advance to confirm inventory.

Online Platform

Online ordering through compliant cross-border medical platforms is a more convenient way, but patients need to choose a platform with drug import qualifications and upload prescriptions and other certificates. The advantage is that it saves time, and some platforms provide professional medication guidance. The disadvantage is that the authenticity of the platform needs to be carefully verified to avoid encountering fake websites.

Please keep the outer packaging and drug purchase receipt when purchasing to facilitate customs inspection. There are currently no generic drugs on the market, and low-price channels have higher risks. Patients need to be more vigilant.

What side effects will Valdostat (Vafseo) cause?

Valostat may cause a variety of adverse reactions, and timely intervention can effectively control the risk.

Hypertension and thrombotic events

Some patients may experience elevated blood pressure and need to monitor and adjust antihypertensive drugs daily. If chest pain, unilateral limb swelling or dyspnea occurs, it may indicate thrombosis, and the drug needs to be stopped immediately and medical treatment required. Mild risk of blood clots can be prevented with low-dose anticoagulants, but only after a doctor's evaluation.

Hepatotoxicity and treatment measures

Some patients may experience elevated ALT/AST, and liver function needs to be tested monthly for the first 6 months of treatment. If ALT/AST is >3 times the normal value or accompanied by jaundice, the drug needs to be permanently discontinued and hepatoprotective treatment such as glycyrrhizic acid preparations should be received. For mild abnormalities, the dose can be maintained and the monitoring interval shortened to every 2 weeks.

Some patients may suffer from diarrhea. It is recommended to eat small meals frequently, avoid high-fiber foods, and take montmorillonite powder orally if necessary. If black stool or vomiting blood occurs, it may indicate gastrointestinal bleeding and requires emergency medical treatment and suspension of medication. Mild nausea can be relieved with ginger slices or vitamin B6.

Precautions for Vafseo

Scientific medication can reduce the chance of adverse reactions. Patients need to pay attention to the following matters.

Contraindications for combined use of drugs

Avoid taking valostat with iron-containing drugs such as ferrous succinate at least 1 hour apart. When combined with simvastatin, the statin dose should be limited to 20 mg/day. When using diuretics such as furosemide, blood potassium levels need to be monitored to prevent hypokalemia.

Scope of contraindications for special groups

It is contraindicated in patients with chronic kidney disease, acute liver disease or cirrhosis who are not receiving dialysis valostat. People who have had myocardial infarction, stroke or venous thrombosis within 3 months should avoid use. Pregnant women should only use it with caution when the benefits are clear, and lactating women should suspend breastfeeding.

Hemoglobin (Hb) needs to be tested every 2 weeks during treatment, and the target value is ≤11g/dL. If the Hb rising rate is >1g/dL/2 weeks, the medication needs to be suspended and the dose reduced to 150mg/day. Those who fail to respond within 24 weeks need to stop taking the drug and re-evaluate the cause.
