Should Delamanid be taken before or after meals?
After a single oral dose of 100mg Delamanid , the peak plasma concentration is 135ng/mL and reaches steady-state concentration after 10-14 days. Plasma exposure to delamanid increases disproportionately with increasing doses. In animal models (dog, rat, mouse), the oral bioavailability of delamanid has been reported to be 35%-60%, and the absolute oral bioavailability in humans is estimated to be between 25% and 47%.

It is recommended to take delamanid with food as food facilitates better absorption compared to first-line anti-tuberculosis drugs which should be taken on an empty stomach. Following oral administration, maximum concentrations are observed at 4-5 hours. The half-life after discontinuation of medication is 38 hours. In early trials, delamanid exposure was not found to be dose-proportional and it plateaued at 300 mg. This may be due to the poor water solubility of the drug and limited absorption at high doses. When administered with a standard meal, human oral bioavailability is increased approximately 2.7-fold compared to fasted conditions because delamanid exhibits poor water solubility.
The original drug Delamani has been launched in China and is included in the scope of Class B medical insurance. Reimbursement is limited to patients with multi-drug-resistant tuberculosis. Specifications The price of each box of 50mg*60 tablets may be around RMB 6,000. The price of the Russian version of the original drug Dramani, 50mg*48 tablets, sold overseas may be around RMB 1,000-2,000 per box (the price may fluctuate due to the exchange rate).
