Delamanid is a newer inhibitor of mycobacterial cell wall synthesis that has received conditional approval from the European Medicines Agency (EMA) for the treatment of multidrug-resistant tuberculosis (MDR-TB). Preclinical and clinical studies have shown that delamanid has higher potency, minimal risk of drug interactions and better tolerability.

Multidrug-resistant tuberculosis(MDR-TB) is an infection caused by Mycobacterium tuberculosis that is resistant to rifampicin and isoniazid. If resistance extends to second-line anti-tuberculosis drugs, such as fluoroquinolones and an injectable drug, it is called extensively drug-resistant tuberculosis (XDR-TB). The coexistence of TB in immunosuppressive conditions (such as AIDS and malnutrition) and the increased incidence of multidrug-resistant and XDR-TB in such settings are common. The long-term treatment with delamanid and the 24-month observational study are a continuation of previous short-term trials to understand treatment outcomes. Patients who received delamanid for ≥6 months had a better prognosis than those who received ≤2 months of treatment. Mortality was significantly lower in the long-term delamanid treatment group.

The original drug Delamani has been launched in China and is included in the scope of Class B medical insurance. Reimbursement is limited to patients with multi-drug-resistant tuberculosis.
