The difference between fenelidone and dapagliflozin
Finerenone (Finerenone) and dapagliflozin each play an important role in the treatment of diabetes and chronic kidney disease, but they have some differences in their mechanisms of action, indications and clinical applications.
Finelidone is an aldosterone receptor antagonist mainly used to treat adult chronic kidney disease (CKD) associated with type 2 diabetes (T2D). Aldosterone is a hormone that plays an important role in the kidneys and is involved in regulating blood pressure and electrolyte balance. However, in people with chronic kidney disease, aldosterone levels may become abnormally elevated, causing further damage to the kidneys. By blocking aldosterone receptors and reducing the effects of aldosterone, fenelinone helps protect kidney function, avoid a sustained decline in eGFR (estimated glomerular filtration rate), and reduce the risk of end-stage renal disease, cardiovascular death, non-fatal myocardial infarction, and hospitalization for heart failure.

Dapagliflozin is aSGLT-2 inhibitor, mainly used to treat heart failure, chronic kidney disease and type 2 diabetes. SGLT-2 is a transporter protein located in the kidneys that is responsible for reabsorbing glucose from the blood back into the body. In patients with diabetes, blood sugar increases due to insufficient secretion or weakened action of insulin. Dapagliflozin reduces the reabsorption of glucose by the kidneys by inhibiting the action of SGLT-2, allowing more glucose to be excreted through the urine, thereby lowering blood sugar levels. Additionally, dapagliflozin has benefits including lowering blood pressure, weight loss, and improving cardiovascular health.
In clinical application, fenelidone and dapagliflozin each have their own unique indications and medication guidelines. Fennelidone is mainly used to treat patients with CKD related to T2D, while dapagliflozin is more widely used to treat heart failure, chronic kidney disease and type 2 diabetes. In addition, these two drugs may also be used for other purposes not listed in this Medication Guide, but only under the guidance of a doctor.
