Nebivolol has good effects in patients with heart failure. It is a safe and effective drug and is widely used in the treatment of hypertension and heart failure. Methods and results

The trial randomly assigned 2128 patients aged ≥70 years with a history of heart failure (hospital admission for heart failure within the previous year or a known ejection fraction of less than or equal to 35%), 1067 of whom received nebivolol (at doses ranging from 1.25 mg once daily to 10 mg once daily) or 1061 who received placebo. The primary study outcome was a composite of all-cause mortality or cardiovascular admissions (time to first event). Analyzes were conducted using the intention-to-treat approach.

The mean follow-up period was 21 months. The mean age was 76 years (SD 4.7), 37% were female, the mean ejection fraction was 36% (35% had an ejection fraction greater than 35%), and 68% had prior coronary artery disease. Mean maintenance was 7.7 mg for nebivolol and 8.5 mg for placebo.

The primary outcome occurred in 332 patients (31.1%) taking nebivolol compared with 375 patients (35.3%) taking placebo [hazard ratio (HR) 0.86, 95% CI 0.74-0.99; P=0.039]. Age, sex, or ejection fraction had no significant effect on the effect of nebivolol on the primary outcome.

Nebivolol, a beta-blocker with vasodilatory properties, is an effective and well-tolerated drug for the treatment of heart failure in the elderly.

Nebivolol side effects

Nebivolol has fewer side effects, with the most common ones being headache, dizziness, fatigue, constipation, diarrhea, and paresthesia. These adverse reactions are usually transient and rarely lead to discontinuation of treatment. Nebivolol does not cause orthostatic hypotension and rarely induces heart failure or severe bradyarrhythmias.
