Acoramidis for ATTR-CM: TTR Stabilizer to Reduce CV Death and Hospitalization
1.Indication Boundary
Acoramidis(Attruby)was FDA-approved in November 2024 for adults with wild-type or hereditary transthyretin amyloidosis cardiomyopathy(ATTR-CM).
Goal:
reduce cardiovascular death;
reduce cardiovascular-related hospitalization;
slow disease progression.
Not intended for:
all amyloidosis types,e.g.AL amyloidosis;
all heart failure patients;
direct clearance of deposited amyloid.
2.Why It Works
In ATTR-CM,unstable TTR dissociates and misfolds,depositing in the myocardium and stiffening the heart.
Acoramidis is a highly selective small-molecule TTR stabilizer:
mimics protective TTR conformation;
stabilizes the TTR tetramer;
reduces dissociation and toxic amyloid generation;
preserves normal TTR transport function.
It works upstream,not by directly dissolving deposits.
3.Clinical Evidence
Based on the phase 3 ATTRibute-CM trial:632 symptomatic ATTR-CM adults,30 months.
Key results:
composite endpoint favored acoramidis over placebo;
~81%survival at 30 months vs~74%with placebo;
mean annual CV hospitalization about 0.3 vs 0.6;
functional and quality-of-life measures improved.
Caveats:
benefit is not cure;
outcomes vary by genotype and disease stage;
requires echo,biomarkers,and genetic testing context.
4.Use and Safety
oral,usually twice daily,swallow whole;
dose per local labeling and prescriber;
monitor cardiac function,renal function,electrolytes,NT-proBNP;
common reactions:mild GI symptoms—diarrhea,epigastric pain;
check CYP interactions;
do not self-stop,up-dose,or combine with other ATTR drugs informally.
