[All names] eplerenone, Inspra, eplerenone, Planep

【Indications】For congestive heart failure after acute myocardial infarction. Used to treat high blood pressure.

【Usage and Dosage】

Usual dosage for adults with congestive heart failure:

Initial dose: 25 mg orally once daily; gradually increase to target dose within 4 weeks as tolerated by the patient.

Target dose: 50 mg PO once daily

Common dosages for adults with high blood pressure:

Eplerenone can be used alone or in combination with other antihypertensive drugs.

Initial dose: 50 mg PO once daily

Maintenance dose: 50 mg orally, 1-2 times daily

Maximum dose: 100 mg/day

Obvious antihypertensive effects appear within four weeks of taking the drug. Patients whose blood pressure responds inadequately to the initial dose may be increased to 50 mg twice daily.

【Adverse reactions】

Common Adverse Reactions: Endocrine and Metabolic: Hyperkalemia ([Heart failure, post-myocardial infarction: >5.5meq/L: 16%; ≥6meq/L: 6%, [Hypertension, >5.5meq/L: 400mg: 9%; dose ≤200mg: ≤1%]), hypertriglyceridemia (1%-15%; dose related)

Central nervous system: dizziness (3%), fatigue (2%); endocrinology and metabolism: hyponatremia (2%; dose-related), proteinuria (1%), gynecomastia (≤1%), hypercholesterolemia (≤1%); gastrointestinal tract: diarrhea (2%) ), abdominal pain (1%); urogenital system: abnormal vaginal bleeding (≤2%), breast pain (men: ≤1%); kidney: elevated serum creatinine (heart failure, post-myocardial infarction: 6%); respiratory system: cough (2%), flu-like symptoms (2%). <1% of patients develop symptoms of angioedema, increased blood urea nitrogen, increased liver enzymes, increased uric acid, and rash.

[Drug Interactions] Significant interactions may exist, requiring dose or frequency adjustment, additional monitoring, and/or the selection of alternative therapies.

Potassium Supplements: Avoid potassium supplements, potassium-containing salt substitutes, potassium-rich diets, or other medications that may cause hyperkalemia (e.g., other potassium-sparing diuretics, nonsteroidal anti-inflammatory drugs). Concomitant use of potassium supplements or potassium-sparing diuretics is contraindicated in the treatment of hypertension.

【Storage】Should be stored at 25°C (77°F); allowed to deviate from 15°C to 30°C (59°F to 86°F).
