The recommended dose of eplerenone for the treatment of congestive heart failure is as follows: Adult patients: Initial dose: 25 mg, oral, once daily; and gradually increase to the target dose (50 mg, oral, once daily) within 4 weeks as tolerated by the patient. 

For adult patients with hypertension, eplerenone can be used alone or in combination with other antihypertensive drugs. Initial dose: 50 mg, oral, once daily; Maintenance dose: 50 mg, oral, 1-2 times daily; Maximum dose: 100 mg/day.

Patients with hypertension usually experience significant antihypertensive effects within four weeks of taking eplerenone. Patients whose blood pressure responds inadequately to the initial dose may be increased to 50 mg twice daily.

Hyperkalemia may occur during treatment with eplerenone, so hyperkalemia needs to be monitored closely and dose reduction or treatment interruption required if necessary. Use with caution in patients with diabetes and heart failure after myocardial infarction (especially those with proteinuria).
