Medication precautions:

1. Digital rectal examination and other examinations for prostate cancer must be performed before patients with benign prostatic hyperplasia (BPH) start treatment with dutasteride, and regularly after treatment.

2. Dutasteride can be absorbed through the skin, so women, children and teenagers must avoid contact with leaking capsules. If you accidentally come into contact with a leaking capsule, wash the affected area immediately with soap and water.

3. No clinical studies of dutasteride have been conducted in patients with liver disease. Therefore, dutasteride should be used with caution in patients with mild to moderate hepatic impairment.

4. Impact on ability to drive and use machines

Based on the pharmacodynamic properties of dutasteride, treatment with dutasteride is not expected to affect the ability to drive or operate machinery.

5. Serum prostate-specific antigen (PSA) concentration is an important indicator for detecting prostate cancer. Generally, when the total serum PSA concentration is greater than 4 ng/mL (using the Hybritech method), further testing is required and prostate tissue biopsy is considered. Physicians should note that a baseline PSA value below 4 ng/mL does not exclude a diagnosis of prostate cancer in patients taking dutasteride. In patients with benign prostatic hyperplasia, and even in patients with prostate cancer, serum PSA levels can be reduced by approximately 50% after 6 months of treatment with dutasteride. Although individual differences exist, approximately 50% reduction in PSA values ​​can be expected across the entire range of baseline PSA values ​​(1.5-10ng/ml). Therefore, when interpreting isolated PSA values ​​in patients who have been treated with dutasteride for 6 months or longer, the PSA value should be doubled and compared to the normal range in patients not taking the drug. Through this correction, the sensitivity and specificity of the PSA test are maintained and its ability to detect prostate cancer is guaranteed. Patients who use dutasteride and whose PSA index continues to rise should be carefully evaluated, including considering factors such as the patient's poor compliance with dutasteride treatment. Within 6 months of stopping treatment, total serum PSA values ​​returned to baseline levels. Despite the effects of dutasteride, the ratio of free PSA to total PSA remained stable. If the clinician chooses to use the free PSA percentage as an adjunct to monitor for the development of prostate cancer in treated patients, no correction of the PSA value is necessary.
