Rivaroxaban用法用量

Rivaroxaban is a new oral anticoagulant indicated for adult patients with non-valvular atrial fibrillation (except atrial fibrillation caused by rheumatic valvular heart disease and atrial fibrillation after heart valve replacement) to reduce the risk of stroke and systemic embolism.

So, what is the dosage of Rivaroxaban?

How to use Rivaroxaban and dosage:

For patients with non-valvular atrial fibrillation, the recommended dose is 20 mg once a day. For patients with low body weight (≤50 kg) and elderly patients (>75 years old), doctors may reduce the dose to 15 mg once a day at the discretion of the doctor. Rivaroxaban 10mg can be taken on an empty stomach, while the 15mg and 20mg doses of Rivaroxaban should be taken with food.

Because Rivaroxaban 10mg has high bioavailability whether taken on an empty stomach or with food, Rivaroxaban 15mg and 20mg have a bioavailability of only 66% in the fasting state. After being fully mixed with food, it can increase the full contact between the drug and the gastric mucosa, and can also prolong the residence time of the drug in the stomach, which is beneficial to gastric absorption. In addition, for patients who cannot swallow the whole tablet, Rivaroxaban can be crushed and taken by nasogastric feeding.

Overdose of Rivaroxaban:

1. Due to the pharmacodynamic properties of Rivaroxaban, overdose may lead to bleeding complications. There is no specific antidote for the effects of Rivaroxaban. If an overdose of Rivaroxaban occurs, consider using activated charcoal to reduce absorption.

If bleeding occurs, take the following steps to manage bleeding:

(1) Appropriate symptomatic treatment, such as mechanical compression, surgery, fluid replacement, and hemodynamic support, and transfusion of blood products or blood components should be considered.

(2) Delay the next administration of Rivaroxaban or terminate treatment at an appropriate time. The average terminal half-life of Rivaroxaban is 7 to 11 hours.
