Adrenocortical carcinoma (ACC) is a relatively rare endocrine cancer with a poor prognosis. Complete surgical resection is currently the preferred treatment method, and surgical methods include open adrenalectomy and laparoscopic adrenalectomy. ASEAN Mitotane A steroid hormone synthesis inhibitor and cytostatic antineoplastic drug, mitotane (mantotan) is the only drug approved for use in patients with end-stage adrenocortical cancer (ACC).

A clinical trial to evaluate the efficacy of ASEAN in advanced ACC and identify prognostic factors. The researchers included a total of 127 patients with advanced ACC who were treated with mitotane monotherapy. Progression-free survival and overall survival (PFS, OS) were assessed. The study results showed that 26 patients (20.5%) experienced objective responses, including 3 patients who had complete remission. Overall, the median PFS was 4.1 months and the median OS was 18.5 months. Multivariable analysis revealed two major prognostic factors: low tumor burden (less than 10 tumor lesions): hazard ratios (HRs) for progression and death of 0.51 and 0.59, respectively; and mitotane initiation at late onset of late relapse: HRs of 0.35 and 0.34, respectively. Thus, 67% of patients with low tumor burden and mitotane initiation ≥360 days achieved clinical benefit (≥180 days of stable disease) after initial diagnosis. Patients achieving mitotan levels >14 mg/l had significantly worse OS.
