来法莫林(Xenleta)的用药注意事项

When using Xenleta, you need to pay attention to QT interval prolongation, Clostridium difficile-related diarrhea, etc., and you need to take the medication under the guidance of a professional physician.

Precautions before taking Xenleta

1. Clear contraindications

Confirm whether the patient has the following contraindications. If so, it must not be used:

(1) Known allergies: allergic to lefamoren, pleuromutilins or any component of this product.

(2) Specific drug interactions: It is prohibited to combine oral lefamoren tablets with drugs that are mainly metabolized by CYP3A4 and can prolong the QT interval (such as pimozide).

2. Assess the patient's condition

(1) Heart condition: Understand whether there is a history of QT interval prolongation, arrhythmia (such as torsade de pointes) or related symptoms.

(2) Liver function: Liver function must be tested. In patients with severe hepatic impairment, the intravenous form requires dose adjustment (every 24 hours instead), while the oral tablet form is not recommended.

(3) Pregnancy and contraception: Conduct pregnancy tests for all women of childbearing potential to confirm that they are not pregnant. They were also informed that they need to take effective contraceptive measures during treatment and within 2 days after the last dose.

(4) Medication list: Check in detail all medications currently taken by the patient (including prescription drugs, over-the-counter drugs and health care products) to identify potential drug interaction risks.

Precautions during the use of Xenleta

1. Strictly follow the usage and dosage

(1) Dosage form and course of treatment: Strictly follow the dosage form, dose and course of treatment prescribed by the doctor. Do not switch, increase or decrease the dose on your own, or stop the drug early.

(2) Oral administration: Tablets should be taken on an empty stomach 1 hour before or 2 hours after a meal. Take the entire tablet with plenty of water and do not crush or chew it.

(3) Treatment of missed doses: If you miss a dose, you can take it immediately if it is more than 8 hours before the next dose; if it is less than 8 hours, skip this time and take the next dose as originally planned. Do not double the dose.

2. Be wary of serious adverse reactions

(1) Cardiac monitoring: If the patient is at risk of QT prolongation or is taking other drugs that can prolong the QT interval, and the use cannot be avoided, electrocardiogram monitoring is recommended.

(2) Identify diarrhea: Once diarrhea occurs, especially watery stools, bloody stools or accompanied by fever and abdominal pain, you should contact your doctor immediately. It may be a symptom of Clostridium difficile infection, which can occur 2 months or even later after taking the medication.

(3) Pay attention to physical reactions: Pay attention to common adverse reactions such as nausea, vomiting, pain at the injection site, rash, headache, etc., and report to the doctor in time.

3. Management of drug interactions

(1) Avoid concomitant use: When taking lefamorelin orally, avoid concomitant use with strong CYP3A inhibitors (such as ketoconazole) or strong inducers (such as rifampicin) to avoid increasing toxicity or reducing efficacy.

(2). Use with caution: Be extra cautious when combined with a variety of antiarrhythmic drugs, antipsychotics, certain antibiotics (such as erythromycin) and tricyclic antidepressants, as QT prolongation effects may be superimposed.

Precautions after taking Xenleta

1. Complete the full course of treatment

Even if the symptoms have improved in advance, the entire course of treatment must be completed as directed by the doctor to completely eliminate bacteria and prevent the recurrence of infection and the emergence of drug-resistant bacteria.

2. Continue to observe adverse reactions

Some adverse reactions (such as diarrhea, abnormal liver function indicators) may appear or persist after stopping the drug. If you have any persistent or new discomfort after stopping this medication, you still need to seek medical attention.

3. Post-medication arrangements for special groups of people

(1) Lactating women: After the last dose, breast milk should be continued to be pumped and discarded for at least 2 days before breastfeeding can be resumed.

(2) Contraceptive measures: Women of childbearing potential should continue to use effective contraception for at least 2 days after the last dose.
