Respimat, Slava, Tiotropium bromide is a long-acting, specific antimuscarinic drug, often referred to as an anticholinergic drug in clinical practice. By binding to muscarinic receptors on bronchial smooth muscle, tiotropium inhibits the cholinergic (bronchoconstrictor) effects of acetylcholine released from parasympathetic nerve terminals. It has similar affinity for muscarinic receptor subtypes M1-M5. In the respiratory tract, tiotropium competitively and reversibly inhibits M3 receptors, causing smooth muscle relaxation. This effect is dose-dependent and can last for more than 24 hours. The long effect may be due to its very slow dissociation from the M3 receptor, and its dissociation half-life is significantly longer than that of ipratropium bromide.

1. Overview In a clinical trial in which 906 patients received tiotropium bromide for 1 year, the most frequently occurring adverse reaction was dry mouth. Dry mouth occurs in approximately 14% of patients. Dry mouth reactions are usually mild and resolve with continued treatment.

Listed below are the reporting rates of this adverse reaction, which are derived from clinical trials of patients receiving tiotropium bromide for 1 year (according to WHO organ system classification) Adverse reaction frequency Systemic allergic reactions: Rare (i.e., >0.1%, <1%) Gastrointestinal system disorders: dry mouth Very common (i.e., >10%) Constipation Common (i.e., >1%, <10%) Heart rate and arrhythmias: Tachycardia Rare (i.e., >0.1%, <1%) Palpitations Rare (i.e., >0.1%, <1%).

The following special groups should pay attention when taking Tiotropium Bromide: Respimat, Slava, and Tiotropium Bromide: 1. Elderly patients can use Tiotropium Bromide at the recommended dose. 2. Patients with renal insufficiency can use tiotropium bromide at the recommended dose. However, in patients with moderate to severe renal impairment (creatinine clearance ≤50 ml/minute), the use of tiotropium should be closely monitored, as with other drugs that are primarily excreted by the kidneys. 3. Patients with liver dysfunction can use tiotropium bromide at the recommended dose. 4. There is no experience in using tiotropium bromide in pediatric patients, so patients younger than 18 years old are not recommended to use Tiotropium bromide.
