布地奈德注意事项

It is a glucocorticoid with highly effective local anti-inflammatory effects. The following are precautions in the package insert for budesonide:

1. Adrenal suppression: May lead to corticosteroid excess or hypothalamic-pituitary-adrenal (HPA) axis suppression, especially in young children or patients receiving high-dose therapy for a long time. Special caution is required when patients are transferred from systemic corticosteroids to inhaled products due to possible adrenal insufficiency or steroid withdrawal, including increased allergic symptoms. Adult patients taking ≥20 mg of prednisone (or equivalent) daily may be most susceptible.

2. Immunosuppression: Long-term use of corticosteroids may also increase the incidence of secondary infections, mask acute infections (including fungal infections), prolong or aggravate viral infections, or limit the response to vaccines. Avoid exposure to chickenpox or measles, especially if you are not immune. Avoid use or use with caution in patients with latent/active pulmonary tuberculosis, untreated bacterial or fungal infections (local or systemic), viral or parasitic infections, or ocular herpes simplex. The Canadian label prohibits use in patients with active or quiescent tuberculosis or untreated bacterial, fungal, or viral infections.

3. Eye diseases: Use with caution in patients with cataracts or glaucoma; long-term use can lead to increased intraocular pressure, glaucoma and cataracts. Consider routine eye exams for chronic users or patients who report changes in vision.

4. Pediatrics: Avoid using higher than recommended doses of the drug; when corticosteroids are administered to pediatric patients, growth rates may be reduced, even at recommended doses via the nasal route (monitor growth).

5. Fetal toxicity: Like other glucocorticoids, it caused various types of malformations (cleft palate, skeletal deformities) in animal tests. However, the correlation between the data from animal experiments and humans has not yet been demonstrated. Until more experience is gained, pregnant women should not use this product unless there are special considerations.
