It is a RANK ligand (RANKL) inhibitor. On November 18, 2010, it was approved to prevent bone-related events caused by cancer that has spread to the bones. In June 2013, it was approved to treat adults and adolescents with giant cell tumor of bone (GCTB), a rare and usually noncancerous tumor. Denosumab has been approved for marketing in my country, and many domestic patients also call it denosumab.

Giant cell tumor of bone is one of the more common primary bone tumors. If it is not completely treated, young patients may need continuous treatment for the rest of their lives. In severe cases, it may cause bone damage leading to fractures, joint dysfunction and even amputation, which will cause great pain to the patient both physically and psychologically. The launch of denosumab undoubtedly provides a new treatment option for these patients and brings new hope.

What are the precautions for denosumab?

Understanding the precautions for denosumab can ensure the normal effect of the drug and avoid unnecessary accidents. Denosumab is an injection and can only be used for subcutaneous injection, not intravenous infusion, intramuscular infusion or intradermal injection. Inject 120 mg subcutaneously into the upper arm, upper thigh, or abdomen every 4 weeks.

Because hypocalcemia may occur, patients who are prone to hypocalcemia and mineral metabolism imbalance, such as those with a history of thyroid surgery, hypoparathyroidism, malnutrition, parathyroid surgery, small bowel resection, and severe renal insufficiency, need to closely monitor their creatinine and mineral levels, and instruct such patients to pay attention to the symptoms of hypocalcemia and to supplement with adequate amounts of calcium and vitamin D.

The use of denosumab may also increase the risk of infection, especially in patients who are also taking immunosuppressives or have compromised immune systems. Such patients need to be evaluated by a doctor before deciding whether to take it.
