Lytenava: The First FDA-Approved Ophthalmic Bevacizumab Formulation

  1.Drug Overview

  Lytenava(generic name:bevacizumab-vikg;development code:ONS-5010)is an anti-vascular endothelial growth factor(VEGF)ophthalmic formulation developed by Outlook Therapeutics,Inc.,a U.S.-based biopharmaceutical company.On July 24,2026,the U.S.Food and Drug Administration(FDA)officially approved Lytenava for the treatment of neovascular age-related macular degeneration(nAMD),commonly known as wet AMD.This marks the first time the FDA has approved a dedicated ophthalmic formulation of bevacizumab,ending more than two decades of off-label use of this drug in retinal disease treatment.

  Prior to its FDA approval,Lytenava had already received marketing authorization in the European Union and the United Kingdom.Under the U.S.Biologics Price Competition and Innovation Act,Lytenava is expected to enjoy a 12-year market exclusivity period.

  2.Disease Background and Unmet Medical Need

  Age-related macular degeneration(AMD)is a chronic,progressive retinal disease that primarily affects the macula in older adults and is one of the leading causes of severe vision loss worldwide.The macula is responsible for central vision,reading,and fine visual tasks.Damage to this area results in blurred or dark spots in the central visual field.

  AMD is classified into dry and wet forms.Although wet AMD accounts for a smaller proportion of cases,it progresses rapidly and is characterized by abnormal neovascularization beneath the retina.These fragile new blood vessels are prone to leakage and hemorrhage,leading to macular edema and rapid deterioration of central vision.VEGF plays a critical role in driving this pathological angiogenesis,making VEGF pathway inhibition the cornerstone of wet AMD treatment.

  3.Development History and Regulatory Journey

  Bevacizumab was originally developed as an oncology drug administered via intravenous infusion for various malignancies.However,owing to its potent anti-VEGF activity,ophthalmologists worldwide began using it off-label for retinal diseases such as wet AMD starting in the early 2000s.Despite its widespread clinical use,bevacizumab never held a formal ophthalmic indication.

  Outlook Therapeutics addressed this gap by developing Lytenava as a purpose-built ophthalmic formulation of bevacizumab,with dedicated manufacturing processes,dosage form design,and quality control systems tailored for intravitreal injection.

  The regulatory path to FDA approval was challenging.In 2023,the FDA rejected the first application citing manufacturing issues and the need for additional confirmatory clinical evidence.In August 2025,a second rejection was issued with the primary concern being insufficient evidence of efficacy.Outlook Therapeutics subsequently supplemented pivotal clinical data and resubmitted the application,ultimately receiving approval in July 2026.

  4.Mechanism of Action

  The active ingredient of Lytenava,bevacizumab gamma,is a recombinant humanized IgG1 monoclonal antibody that specifically binds to VEGF-A,preventing its interaction with receptors VEGFR-1 and VEGFR-2 on vascular endothelial cells.By inhibiting VEGF signaling,Lytenava reduces abnormal retinal neovascularization,decreases vascular permeability,and alleviates fluid leakage and macular edema,thereby helping to preserve and improve central visual function.

  5.Key Clinical Trial Data

  FDA approval was supported by comprehensive evidence from four Phase III clinical trials:NORSE ONE,NORSE TWO,NORSE THREE,and NORSE EIGHT.

  NORSE TWO was a randomized,masked,active-controlled Phase III study enrolling 228 patients with wet AMD,using ranibizumab as the comparator.Results demonstrated that Lytenava significantly outperformed the control group in best-corrected visual acuity(BCVA)improvement:in the intent-to-treat population,41.7%of Lytenava-treated patients achieved a BCVA gain of 15 or more letters,compared to 23.1%in the ranibizumab group(P=0.0052).

  NORSE THREE confirmed that the safety profile of Lytenava was manageable,with overall adverse event rates comparable to other anti-VEGF ophthalmic therapies.NORSE EIGHT further demonstrated superiority of Lytenava over ranibizumab in BCVA improvement at Week 12,providing additional efficacy support for the FDA approval.

  6.Dosing and Clinical Significance

  Lytenava is administered via intravitreal injection by a qualified ophthalmologist.The recommended dose is 1.25 mg(0.05 mL),typically given approximately every four weeks.

  As the first FDA-approved dedicated ophthalmic bevacizumab formulation,Lytenava represents a significant milestone in retinal disease treatment.It formally ends the era of off-label bevacizumab use in ophthalmology and provides patients with wet AMD a standardized,regulatory-approved anti-VEGF treatment option.With the global aging population driving an increasing prevalence of wet AMD,Lytenava is positioned to play an important role in addressing this growing therapeutic need.
