Tucatinib HER2 TKI: With Trastuzumab + Capecitabine, Diarrhea and Hepatotoxicity Monitoring

　　1.Targeted Drug,Not Chemo

　　Tucatinib is an oral small-molecule HER2 tyrosine kinase inhibitor.

　　Chemotherapy acts broadly on dividing cells;tucatinib selectively enters the cell,binds the HER2 kinase ATP site,blocks autophosphorylation,and interrupts downstream proliferation/survival signaling.

　　Key limits:

　　targeted therapy,not cytotoxic chemo;

　　HER2-selective,less EGFR spillover;

　　does not replace trastuzumab;

　　“targeted”does not mean no toxicity.

　　2.Who It Is For

　　Indication boundaries set by oncology:

　　HER2-positive advanced/metastatic breast cancer:adults,unresectable or metastatic,prior anti-HER2 regimen in metastatic setting,with trastuzumab+capecitabine;brain metastases allowed.

　　HER2-positive RAS wild-type metastatic colorectal cancer:with trastuzumab after fluoropyrimidine/oxaliplatin/irinotecan failure,accelerated approval.

　　Maintenance extension 2026:tucatinib+trastuzumab+pertuzumab after induction.

　　Not appropriate:

　　HER2-negative;

　　unconfirmed molecular subtype;

　　self-initiation without prescription;

　　pregnancy;severe hepatic risk needs dose change.

　　3.Why Trastuzumab+Capecitabine

　　Three complementary mechanisms:

　　tucatinib:intracellular HER2 TKI;

　　trastuzumab:extracellular HER2 antibody;

　　capecitabine:chemo backbone converted to 5-FU.

　　Dosing:

　　300mg orally twice daily~every 12h;

　　with or without food;

　　swallow whole;

　　if miss>6h skip,no double dose.

　　4.Safety

　　Hepatotoxicity(boxed warning)

　　Monitor ALT,AST,bilirubin before and every 3 weeks.

　　Hold/reduce/stop by severity.

　　Diarrhea

　　Most common meaningful toxicity.

　　Persistent watery diarrhea,dehydration,hypotension,AKI→hold and seek care.

　　Embryo-fetal toxicity

　　Contraception in reproductive-potential patients;no breastfeeding.

　　Interactions

　　CYP2C8/CYP3A substrate:

　　avoid strong CYP2C8 inhibitors;

　　avoid strong CYP3A inducers;

　　strong CYP3A inhibitors need dose review;

　　watch P-gp substrates.

　　5.Common Misconceptions

　　targeted drug still needs HER2 confirmation;

　　HER2“mutation”wording is imprecise in breast cancer;

　　not chemo-free of risk;

　　severe diarrhea must not be ignored.
