Vigabatrin is not usually used to treat grand mal epilepsy. Grand mal epilepsy is currently called generalized tonic-clonic seizure, which is the most obvious form of epileptic seizure. It is mainly characterized by loss of consciousness and clonus after bilateral tonicity. Antiepileptic drugs can be used to treat the seizures after they have stopped.

It is mainly used as an adjuvant treatment for adults with refractory complex partial seizures (rCPS). It was first approved in the United Kingdom in 1989, and the Food and Drug Administration approved its use in the United States in 2009.

Partial seizures of epilepsy include simple partial seizures, complex partial seizures, and partial seizures followed by generalized seizures, all of which can be treated with Vigabatrin.

However, Vigabatrin is generally not used to treat generalized seizures of epilepsy, including generalized tonic-clonic seizures (grand mal seizures), absence seizures, tonic seizures, clonic seizures, atonic seizures, etc. It may not play a good therapeutic role and has poor efficacy.

Vigabatrin clinical

In one study, 982 observations were made on 747 patients, divided into vigabatrin and placebo groups. Patients treated with Vigabatrin had significantly less seizure frequency than those treated with placebo, and were more likely to experience a reduction of 50% or more. Vigabatrin reduces seizure frequency in patients with drug-resistant partial epilepsy.

There are currently insufficient data to describe the risk-benefit balance of vigabatrin and carbamazepine as monotherapy in the treatment of epilepsy.

Given the high prevalence of visual field defects in observational studies, prescribing Vibatrin monotherapy for the treatment of epilepsy should be used with caution and should not be considered a first-line option. The patient's visual field needs to be evaluated frequently if necessary, as visual disturbances often appear months to years after treatment.

How is generalized tonic-clonic disease treated?

When a generalized tonic-clonic seizure occurs, the seizure should be controlled as soon as possible. The patient should immediately lie on his side and turn his head to one side. Do not press the patient's limbs hard during convulsions to avoid fractures.

At the same time, pre-hospital first aid is required. If it cannot be stopped on its own, rapid antispasmodic drugs such as diazepam can be used under the guidance of a doctor. At the same time, attention should be paid to maintaining smooth breathing and protecting the functions of the brain and other important organs.

After the seizure has stopped, anti-epileptic drugs can be used for treatment to control the seizure as much as possible and improve the patient's prognosis. Commonly used drugs include phenytoin, carbamazepine, valproic acid, topiramate, gabapentin, levetiracetam, etc.
