What are the contraindications for Alprazolam?
Alprazolam is a central nervous system depressant. Although it is widely used in anxiety, panic disorder and anxiety insomnia, it is not suitable for use by all people. Because its pharmacological effects directly affect brain neurotransmitter activity, it may cause serious adverse reactions or complications in some special groups, and doctors' instructions must be strictly followed.
First, alprazolam is contraindicated in people allergic to benzodiazepines. If the patient has experienced rash, dyspnea, or allergic reaction to diazepam, lorazepam and other drugs, he should avoid using alprazolam to prevent cross-allergy. Secondly, this drug is contraindicated in patients with myasthenia gravis because alprazolam can aggravate muscle relaxation and cause respiratory depression or motor dysfunction. Likewise, patients with severe respiratory insufficiency or chronic obstructive pulmonary disease (COPD) should avoid use. The drug may inhibit the respiratory center and aggravate hypoxia.
For patients with severe liver or renal impairment, the metabolism and excretion capacity of alprazolam is reduced, which can easily lead to drug accumulation and cause drowsiness, consciousness disorder or drug poisoning. If such patients require medication, they should start at very low doses under strict medical supervision. Patients with acute angle-closure glaucoma should also avoid taking alprazolam because the drug may increase intraocular pressure and worsen the condition.
Pregnant and breastfeeding women are another high-risk group. Alprazolam crosses the placental barrier and is excreted in breast milk, potentially causing respiratory depression, somnolence, or a withdrawal syndrome in neonates. The FDA lists it as a pregnancy risk category D drug, which means it has potential fetal risks. Therefore, use during pregnancy should be avoided unless the benefits clearly outweigh the risks. In addition, taking medication during lactation may cause sedation and feeding difficulties in the baby. You should consult your doctor before deciding whether to stop medication or breastfeeding.
