How long does it take for pirfenidone (Asre) to take effect and suggestions for treatment course observation?
Pirfenidone is an anti-fibrotic drug used for idiopathic pulmonary fibrosis (IPF). Its main function is to inhibit the expression of fibrosis-related factors in lung tissue, thereby delaying the progression of the disease. Generally speaking, it takes some time for patients to notice the effects of pirfenidone after they start taking it. Clinical studies have shown that after most patients continue to take the drug for about 2–3 months, signs of slowing down the lung capacity or lung diffusion capacity can be seen in pulmonary function tests, and it may take longer to significantly improve symptoms such as shortness of breath and cough.
In the early stages of taking the drug, it is usually recommended to gradually increase the dose of pirfenidone according to the guidance of a physician to reduce the occurrence of side effects such as gastrointestinal discomfort, rash, or photosensitivity reactions. The standard dose is usually 267 mg three times daily, gradually increasing to 801 mg three times a day, for a total dose of approximately 2403 mg daily. During the dose adjustment period, patients need to closely observe their own reactions and regularly review liver function and blood routine to ensure safety and tolerability.

In terms of treatment course observation, doctors usually recommend conducting pulmonary function tests, including vital capacity (FVC) and diffusion capacity of the lungs (DLCO), every 3 months to evaluate the efficacy of the drug. At the same time, a comprehensive judgment is made based on the patient's symptom changes, such as dyspnea, cough frequency and activity tolerance. If necessary, imaging examinations, such as high-resolution CT (HRCT), can be used to observe the progression of pulmonary fibrosis to adjust the course of treatment or evaluate whether to continue taking medication.
The safety of long-term use of pirfenidone also needs attention. Although most patients tolerate it, there are still risks such as abnormal liver function, rash, and photosensitivity reactions. Therefore, patients should strictly follow the doctor's instructions and do not stop taking the medicine or change the dosage at will. If obvious discomfort or adverse reactions occur, seek medical attention promptly. On the premise of following the standard treatment course and observation recommendations, pirfenidone can effectively delay the progression of pulmonary fibrosis and improve patients' quality of life.
