Does Secukinumab damage the liver?

Secukinumab (Secukinumab) is a human monoclonal antibody against interleukin-17A. It is used as an immunosuppressant for the treatment of moderate to severe plaque psoriasis. Secukinumab is not associated with elevated serum enzymes or specific acute liver injury events during treatment, but can lead to reactivation of hepatitis B in susceptible individuals.

The hepatotoxicity of secukinumab has not been shown, but its mechanism of action suggests that it may lead to certain reactivation conditions of hepatitis B and may induce immune-mediated hepatitis. Because of its potential to cause HBV reactivation, patients receiving secukinumab should be screened for HBsAg and anti-HBc. Patients with HBsAg in their serum should receive prophylactic treatment with oral nucleoside analogues (such as tenofovir or entecavir) to prevent HBV reactivation. Patients without HBsAg but with anti-HBc (with or without anti-HBs) are at lower risk for significant reactivation and may require periodic active monitoring of serum HBV DNA and early intervention if viral DNA reappears or serum levels increase.
