How to take Rivaroxaban

Rivaroxaban is administered orally with a half-life of 5-9 hours (may be longer as 11-13 hours in the elderly), with doses ranging from 2.5 mg twice daily to 20 mg once daily, and does not require monitoring. Rivaroxaban is highly protein bound. Doses are generally fixed, no monitoring is required, and the drug does not have an established therapeutic range.

1. Non-valvular atrial fibrillation-Stroke prevention: 20 mg once daily, taken with dinner.

(1) Treatment of acute venous thromboembolism (VTE): 15 mg twice daily with food for three weeks; then 20 mg once daily with food.

(2) VTE primary prevention: 10 mg once daily with or without food.

2. Patients with renal impairment have a higher risk of thrombosis and bleeding than patients with normal renal function. Rivaroxaban is not recommended for VTE prophylaxis in patients with end-stage renal disease (ESRD) who are receiving dialysis and have a creatinine clearance < 30 mL/min.

(1) Treatment and prevention of VTE recurrence: 15 mg twice daily (taken with food) for 21 days, then 20 mg once daily (taken with food). If treatment continues for more than six months, the dose can be reduced to 10 mg once daily.

(2) VTE prophylaxis in surgical patients: 15 mg twice daily (with food) for 21 days, followed by 20 mg once daily (with food). If treatment continues for more than six months, the dose can be reduced to 10 mg once daily. Duration varies from 12-35 days, depending on the type of surgery.

(3) Thromboprophylaxis in patients with acute illness: 10 mg once daily.

(4) Stroke prevention in non-valvular atrial fibrillation: 20 mg once a day with dinner (CrCl>50mL/min); or 15 mg once a day with dinner (CrCl≤50mL/min).

3. If you have used warfarin before, once the INR is <3, stop warfarin immediately and start using rivaroxaban. If you have used low molecular weight heparin before, start using rivaroxaban 0-2 hours before the next scheduled evening dose, and omit the administration of other anticoagulants. If an unfractionated heparin infusion was previously continued, stop the infusion and initiate rivaroxaban simultaneously.

4. Rivaroxaban should be stopped at least 24 hours before surgery. It is recommended to avoid using rivaroxaban for patients with a body mass index >40kg/m or a weight >120kg.
