Panitumumab is indicated for the treatment of patients with metastatic colorectal cancer after failure of other cytotoxic drugs, including chemotherapy with fluoropyrimidine, oxaliplatin, and irinotecan. Panitumumab is the first fully human monoclonal antibody targeting epidermal growth factor receptor (EGFR). EGFR is a transmembrane glycoprotein that belongs to the type I receptor tyrosine kinase subfamily. Although EGFR is expressed in normal cells, overexpression of EGFR has been detected in many human cancers, including cancers of the colon and rectum.

The interaction of EGFR with its normal ligands leads to the phosphorylation and activation of a series of intracellular proteins, which in turn regulates the transcription of genes related to cell growth and survival, movement and proliferation. Signaling through EGFR leads to activation of the wild-type KRAS gene, but the presence of activating somatic mutations of the KRAS gene in cancer cells can lead to dysregulation of the signaling pathway and resistance to EGFR inhibitor treatment. Non-clinical studies have shown that the binding of panitumumab to EGFR prevents ligand-induced receptor autophosphorylation and activation of receptor-related kinases, thereby inhibiting cell growth, inducing apoptosis, reducing the production of pro-inflammatory cytokines and vascular growth factors, and the internalization of EGFR.
