2. Indications:
Opicapone can be used as an adjunct to levodopa/carbidopa in patients with Parkinson's disease (PD) who experience "off" attacks.
3. Usage and dosage:
1. Recommended dose: The recommended dose of Opicapone is50 mg, taken orally once a day before going to bed. Patients should not eat 1 hour before taking this product and at least 1 hour after taking this product.
2. Dosage adjustment:
For patients with moderate hepatic impairment (Child-Pugh B), the recommended dose of Opicapone is 25 mg orally once daily at bedtime; patients with severe hepatic impairment (Child-Pugh C) should avoid using Opicapone.
3. Medication discontinuation and missed doses:
When discontinuation is prolonged, monitor patients and consider adjustment of additional dopaminergic therapy as needed. If a dose of Opicapone is missed, the next dose should be taken at the scheduled time the next day.
4. Adverse reactions:
In clinical trials, the most common adverse reactions (incidence of at least4%) of opicapone were dyskinesia, constipation, elevated blood creatine kinase, hypotension/syncope, and weight loss.
5. Storage:
Opicapone is available as a capsule and is stored at temperatures below30°C (86°F).
6. Taboo:
Opicapone is contraindicated in patients concurrently taking non-selective monoamine oxidase (MAO) inhibitors and in patients with pheochromocytoma, paraganglioma, or other catecholamine-secreting tumors.
7. Mechanism of action:
Opicaponeis a selective and reversible inhibitor of catechol-O-methyltransferase (COMT). COMT catalyzes the transfer of the methyl group of S-adenosyl-L-methionine to the phenolic group of a substrate containing a catechol structure. The physiological substrates of COMT include dopa, catecholamines (dopamine, norepinephrine, and epinephrine) and their hydroxylated metabolites. When carbidopa blocks the decarboxylation of levodopa, COMT becomes the primary metabolic enzyme of levodopa, catalyzing its metabolism to 3-methoxy-4-hydroxy-L-phenylalanine (3-OMD).
8. Special groups:
Opicaponeis often administered concomitantly with levodopa/carbidopa, which is known to cause visceral and skeletal malformations in rabbits. The developmental and health benefits of breastfeeding should be considered together with the mother's long-term clinical needs and any potential adverse effects on the breastfed infant over the long term or potentially.
