What are the precautions for taking Omarigliptin?

Patients should strictly follow the diet and exercise regimen prescribed by their doctor during treatment with Omarigliptin (Omarigliptin). This drug may cause symptoms of hypoglycemia. Be careful when patients are working at heights, driving a car, or operating dangerous machinery. If symptoms of hypoglycemia occur, please eat foods or drinks containing sufficient sugar (sugar, glucose, soft drinks, etc.). If the patient is concomitantly takingalpha-glucosidase inhibitors (drugs that delay sugar absorption), take glucose to prevent any hypoglycemia.

Patients or conditions that may lead to hypoglycemia include pituitary or adrenal insufficiency, malnutrition, starvation, irregular dietary intake, inadequate dietary intake or debilitated state, strenuous muscle activity, excessive alcohol consumption, and the elderly. Before starting ologliptin, assessment of renal function should be recommended. When ologliptin is used with a sulfonylurea or insulin (drugs known to cause hypoglycemia), lower doses of the sulfonylurea or insulin may be required to reduce the risk of hypoglycemia. If a patient suspects pancreatitis, ologliptin should be discontinued immediately and appropriate therapy initiated. Dosage adjustments should be recommended in patients with moderate or severe renal impairment and in patients with ESRD. If an allergic reaction is suspected, ologliptin should be discontinued.

For women who are pregnant or may become pregnant, dosing should be considered only when the benefits of treatment outweigh the risks. In animal studies (rats), oral administration of 100 mg/kg/day (an exposure equivalent to approximately 25 times the clinical dose of 645 mg/week) resulted in decreased fetal weight, a mild increase in the number of fetal ribs, and a decrease in the number of sacrococcygeal vertebrae. Consider continuing or discontinuing breastfeeding, taking into account the therapeutic benefits of breastfeeding.
