1. Indications and Usage
Teriflunomide is a pyrimidine synthesis inhibitor indicated for the treatment of relapsing forms of multiple sclerosis (ms)) in adults, including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease.

Five. Warnings and Precautions
1.Taking cholestyramine or activated charcoal for 11 days can accelerate the elimination of teriflunomide.
2.Teriflunomide may decrease white blood cells. Before initiating teriflunomide, obtain a recent complete blood count and monitor for signs and symptoms of infection. If serious infection occurs, consider suspending teriflunomide treatment and do not use teriflunomide in patients with active infection.
3.If the patient develops allergic reactions, angioedema, Stevens-Johnson syndrome, toxic epidermal necrolysis, drug reactions with eosinophilia, and systemic symptoms, teriflunomide should be discontinued and rapid elimination initiated.
4.If the patient develops symptoms consistent with peripheral neuropathy, evaluate the patient and consider discontinuing teriflunomide.
5.Teriflunomide may increase blood pressure. Blood pressure is measured at the beginning of treatment and monitored during treatment.
6. Adverse reactions
The most common adverse reactions (≥10%and greater than placebo≥2%):Headache, diarrhea, nausea, alopecia, ALT elevation.
7. Drug interactions
Drugs metabolized by CYP2C8 and OAT3 transporters:Monitor patients as teriflunomide may increase exposure to these drugs.
Teriflunomide may increase the exposure of ethinyl estradiol and levonorgestrel, choose an appropriate oral contraceptive.
Drugs metabolized byCYP1A2:Monitor patients as teriflunomide may reduce exposure to these drugs.
Warfarin:MonitorINR as teriflunomide may decrease INR.
Drugs metabolized by BCRP and OATP1B1/B3 transporters:Monitor patients as teriflunomide may increase exposure to these drugs.
Rosuvastatin:The dose of rosuvastatin in patients taking teriflunomide should not exceed10 mg once daily.
8. Mechanism of action
The exact mechanism by which teriflunomide works to treat multiple sclerosis is unknown. Teriflunomide is known to prevent pyrimidine synthesis by inhibiting the mitochondrial enzyme dihydroorotate dehydrogenase, which may be related to its immunomodulatory effects in multiple sclerosis.
