Guidelines for use and dosage recommendations for trifluridine and tipiracil (Lancever)

Trifluridine and tipiracil (Lonsurf) have been approved as a treatment for metastatic colorectal cancer. This drug provides an effective treatment option for adult patients whose disease continues to progress despite treatment or who are unable to receive certain treatments. At the same time, it is also suitable for patients with metastatic gastric cancer who have previously received treatment.

Trifluridine and tipiracil are available in tablet form in two different strengths: 15 mg trifluridine/6 mg tipiracil and 20 mg trifluridine/8 mg tipiracil. To ensure the safety and effectiveness of the medication, the treatment process must be carried out under the guidance and supervision of an experienced oncologist, and this medication is only available with a prescription. Doctors will determine the best dose based on the patient's body surface area. The usual recommended dose is 35 mg of trifluridine per square meter of body surface area.

The treatment cycle is 28 days, and patients need to take the drug twice a day for the first five days. Then stop taking it for two days, then take it twice a day for five more days. After completing this 28-day cycle, the next treatment cycle will begin. Patients should continue treatment as long as they are benefiting from treatment and the side effects are tolerable. However, if the cancer progresses or serious side effects occur, treatment will need to stop.

The doctor will obtain a complete blood cell count before and on the 15th day of each cycle when the patient takes the drug; if discomfort occurs, the dose can be adjusted under the guidance of the doctor, and a maximum of 3 dose reductions are allowed. Patients who cannot tolerate the oral dose of 20 mg/m2 twice daily should permanently discontinue trifluridine tipiracil tablets. Do not increase your dose after reducing it.
