Entrectinib Medication Guide: Detailed explanation of dosage and administration
Entrectinib (Entrectinib) is an effective drug for the treatment of metastatic non-small cell lung cancer and NTRK gene fusion-positive solid tumors. Its use needs to be accurately adjusted according to the specific conditions of the patient. Before choosing to use entrectinib, doctors will first test the tumor or plasma samples for the presence of ROS1 rearrangements. If tumor tissue cannot be obtained, plasma samples can be used for testing. In addition, doctors will also comprehensively evaluate the patient's left ventricular ejection fraction (LVEF), serum uric acid levels, QT interval, electrolytes and other indicators before treatment.
1. Recommended dosage
For patients with ROS1-positive non-small cell lung cancer, the recommended dosage of entrectinib is 600 mg daily, which can be taken with food or on an empty stomach and continued until the disease progresses or unacceptable toxicity occurs.
For NTRK gene fusion-positive solid tumor patients, the dosage needs to be adjusted according to the patient’s age and body surface area:
For pediatric patients aged 1 month to 6 months, the recommended daily dose is 250 mg/m2;
In pediatric patients over 6 months of age, dosage will be determined based on body surface area;
For adult patients whose body surface area is greater than or equal to 1.5m2, the recommended daily dose is 600mg, and the medication should be continued until the disease progresses or toxic reactions occur.

2. Dosage and management: Entrectinib is available in a variety of dosage forms to meet the needs of different patients, including capsules that can be swallowed whole, capsules that can be prepared into oral suspensions (also suitable for enteral tube administration), and oral pills that can be taken with soft food.
Patients can swallow the capsules directly, or the contents of the capsules can be prepared into an oral suspension, which is especially suitable for patients who are unable to swallow capsules or who require administration through a gastric tube or nasogastric tube. When preparing the suspension, follow the instructions and pour an appropriate amount of the capsule contents into room temperature drinking water or milk, let it sit for 15 minutes before taking it, and add the prescribed amount of water. If the suspension cannot be taken immediately, it can be stored at room temperature for up to 2 hours and should be discarded after this time. For oral pills, place them on one or more spoons of soft food (such as applesauce, pudding, yogurt, etc.) and take them within 20 minutes after placing them, making sure the patient takes the entire package of pills. Please note that the pills should not be used for preparing suspensions or for administration via gastric or nasogastric tubes.
3. Dose adjustment: For patients who experience adverse reactions during treatment with entrectinib, doctors will adjust the drug dose according to the severity; during the first dose reduction, reduce the once-daily dose to two-thirds of the initial dose; during the second dose reduction, reduce the once-daily dose to one-third of the initial dose. Patients who cannot tolerate it after two dose reductions should permanently discontinue entrectinib.
