Fenelide works primarily by blocking excessive activation of the mineralocorticoid receptor (MR). Excessive activation of mineralocorticoid receptors is thought to lead to fibrosis and inflammation, leading to permanent renal structural damage.

Common adverse reactions to fenelidone include hyperkalemia (high potassium levels), hypotension, and hyponatremia (low sodium levels).
