The effects and efficacy of Voranigo (Vorasidenib)

Voranigo (Vorasidenib) is a targeted drug used to treat astrocytoma or oligodendroglioma, specifically for patients with specific mutations in IDH1 or IDH2. These brain cancer tumors often require further medical control after surgery to prevent recurrence and spread of the tumor. Voranigo is available as a once-daily oral tablet, providing patients with convenience while ensuring continuity of treatment.

Astrocytomas and oligodendrogliomas are both gliomas derived from the central nervous system (CNS), and their formation is closely related to genetic mutations. Specifically, mutations in the IDH1 and IDH2 genes cause the differentiation process of normal brain cells to be hindered, so that these cells cannot develop normally, but begin to proliferate uncontrollably, resulting in tumors. Voranigo’s mechanism of action is by inhibiting the activity of IDH1 and IDH2 enzymes and reducing the production of intracellular 2-glutarate (2-HG). 2-HG is a metabolite closely related to tumor development. In the process, Voranigo helps restore normal cell differentiation, thereby inhibiting tumor growth.

In clinical studies,Voranigo has shown significant effects in reducing 2-HG tumor concentrations in patients with IDH1 or IDH2 mutant gliomas. The study showed that patients treated with Voranigo at 0.3 to 0.8 times the highest recommended dose experienced significant reductions in 2-HG concentrations in their tumors compared with untreated controls, with reductions in posterior position ranging from 64% (95% confidence interval: 22%, 88%) to 93% (95% confidence interval: 76%, 98%). This result not only demonstrates the drug's effectiveness in reducing tumor markers, but also provides support for its prospects in clinical applications.

However, althoughVoranigo has shown good efficacy, the exposure-response relationship between its safety and efficacy and the time course of pharmacodynamic response still need further research to improve.
