Mitotane usage and dosage guidelines

Mitotane (trade name: Lysodren) is a drug used to treat adrenocortical cancer. Usually administered orally and should be taken with food to improve absorption. Because mitotane is highly fat-soluble and is mainly stored in fat, patients are advised to take it after meals to reduce gastrointestinal discomfort.

The dosage of mitotane needs to be guided by a doctor based on the patient's specific condition.

For patients with adrenocortical cancer, the initial dose is usually 2-6 grams per day, divided into 3 or 4 doses. Taking it in divided doses can help reduce the side effects of the drug.

Physicians will gradually increase dosage based on patient tolerance and clinical response. For patients with adrenocortical cancer, the dose can be gradually increased to 9-10 grams per day, and some patients may require higher doses. Increasing 2-4 grams every 3-7 days is a common dosage adjustment.

The maximum tolerated dose (MTD) is usually between 2-16g per day, but in most cases is 9-10g per day. If a patient experiences serious adverse reactions, the dose should be reduced until the maximum tolerated dose is reached.

While taking mitotane, patients should have regular blood, liver and kidney function tests to ensure the safe use of the drug.

The doctor will adjust the dosage according to the patient's specific condition to achieve the best therapeutic effect.

Common adverse reactions of mitotane include nausea, vomiting, diarrhea, dizziness, drowsiness, loss of appetite, etc. If serious adverse reactions occur, such as persistent nausea, vomiting, abdominal pain, jaundice, rash, etc., you should stop taking the drug immediately and consult a doctor.

Mitotane may interact with other medications, particularly hormonal and anticoagulant medications. Before using mitotane, you should tell your doctor about all the medications you are taking, including prescription drugs, over-the-counter drugs, vitamins, and herbal supplements.
