Standard usage and dosage of donepezil (Aricept) and suggestions for dosage adjustment for different groups of people
Donepezil is an acetylcholinesterase inhibitor commonly used in the treatment of Alzheimer's disease (AD) and other cognitive dysfunctions. Its main function is to increase acetylcholine levels in the central nervous system by inhibiting the degradation of acetylcholine, thereby improving cognitive function and daily living abilities. Standard dosage is oral administration, once daily, usually after dinner to reduce gastrointestinal discomfort and improve compliance. The tablets can be swallowed whole and are not recommended to be chewed or crushed to ensure the sustained release properties of the drug.
For patients with mild to moderate Alzheimer's disease, the initial dose is usually 5mg daily, maintained for 4 to 6 weeks, and may be increased to 10mg daily based on patient tolerance and efficacy. For moderate to severe patients, doctors may recommend directly maintaining it at 10 mg per day, but the patient's tolerance still needs to be closely observed in the early stages of treatment. Dosage adjustment should be based on the patient's clinical response, cognitive function improvement, and adverse reactions, and avoid adding or subtracting the dosage arbitrarily.

Special groups such as elderly patients or those with liver and kidney dysfunction need to adjust the dose carefully. Generally, for patients with moderately or severely impaired liver function, consideration should be given to starting with 5 mg per day and extending the dose adjustment interval; patients with renal insufficiency also need to assess tolerance based on renal function indicators. For patients who are extremely sensitive or have multiple underlying diseases, dosage adjustment should be more individualized. If necessary, the observation period should be extended and the dosage increased under the guidance of a doctor to reduce the occurrence of adverse reactions.
In children and adolescent patients, the application of donepezil is less, and the prescription and medication guidance of professional doctors must be strictly followed. During the entire treatment process, patients should be regularly followed up on their cognitive function, daily living abilities, and adverse reactions, including nausea, vomiting, diarrhea, dizziness, insomnia, etc. Through scientific dose adjustment and regular monitoring, the therapeutic effect of donepezil can be maximized while ensuring safety.
