In opioid-dependent pediatric patients, including neonates, administration of NARCAN Nasal Spray may cause a sudden and complete reversal of the effects of the opioid, precipitating an acute opioid withdrawal syndrome. Unlike adult opioid withdrawal syndrome, neonatal opioid withdrawal syndrome can be life-threatening (if unrecognized) and should be treated according to a protocol developed by a neonatologist. So what should I do if I develop opioid withdrawal syndrome with naloxone hydrochloride?
Symptoms of opioid withdrawal syndrome after using naloxone hydrochloride
Use of NARCAN nasal spray in patients with opioid dependence may precipitate opioid withdrawal, which may be characterized by the following signs and symptoms: body aches, diarrhea, tachycardia, fever, runny nose, sneezing, piloerection, sweating, yawning, nausea or vomiting, nervousness, restlessness or irritability, tremors or tremors, abdominal cramping, weakness, and increased blood pressure.
In some patients, sudden reversal of opioid overdose may result in aggressive behavior. In newborns, signs and symptoms of opioid withdrawal also include convulsions, excessive crying, and hyperreflexia. Monitor patients for signs and symptoms of opioid withdrawal.
What to do if opioid withdrawal syndrome occurs after using naloxone hydrochloride
In neonates with known or suspected maternal opioid exposure, it is best to avoid sudden induction of opioid withdrawal symptoms and consider the use of alternative naloxone-containing products that can be dosed based on body weight and titrated to onset of effect. Additionally, considering the availability of alternative naloxone-containing products than NARCAN nasal spray is more appropriate in settings where the primary concern is infants at risk for opioid overdose.
Naloxone hydrochloride crosses the placenta and may cause withdrawal in the fetus as well as in the opioid-dependent mother. After use of NARCAN nasal spray, the fetus should be evaluated for signs of distress. Careful monitoring is required until the fetus and mother are stable.
