Gout is a recurring inflammatory disease caused by increased purine biosynthesis and metabolism, excessive uric acid production or poor uric acid excretion, resulting in elevated uric acid in the blood. Urate crystals are deposited in joint synovium, bursa, cartilage and other tissues. Febuxostat is a new type of xanthine oxidase inhibitor that reduces uric acid levels by inhibiting the synthesis of uric acid in the body.

How effective is febuxostat for gout patients?

A multicenter, double-blind, randomized phase II clinical study evaluated the safety and efficacy of febuxostat (febuxostat, feburic) in gout. A total of 136 male and 17 female gout patients were randomly assigned to receive placebo or febuxostat (febuxostat, feburic) (40, 80 or 120mg/d). After 4 weeks, the test found that the serum uric acid concentration of patients in each dose group of febuxostat (febuxostat, feburic) was significantly lower than before treatment. According to the dose, each group was from low to high. The average reduction was 37%, 44% and 59% respectively, while patients in the placebo group only reduced by 2%; the vast majority of patients persisted in completing the trial. The incidence of adverse reactions in the febuxostat (febuxostat, feburic) and placebo groups was similar, and most of these adverse reactions were mild and self-limiting, with common ones including diarrhea, pain, back pain, headache and joint pain.

Febuxostat (febuxostat, feburic) works by reducing blood urate concentration. It is completely absorbed after oral administration and has a high utilization rate. Food and antacids have no significant effect on its absorption. Unlike allopurinol, its structure is a non-purine analogue, so it is selective in inhibiting xanthine oxidase and has little effect on other enzymes in purine or pyrimidine metabolism. This means that febuxostat is less likely to be affected by other factors and has a stable effect.
