(Defibrotide) is a drug containing the active substance defibrotide. It is used to treat severe veno-occlusive disease (VOD) in patients undergoing hematopoietic (blood) stem cell transplantation.

Defibrotide is currently recognized as the most promising new drug for the treatment of hepatic veno-occlusive disease (HVOD) after hematopoietic stem cell transplantation (HSCT). Based on its anticoagulant, fibrinolysis-promoting, and anti-inflammatory effects, foreign researchers first explored its application in the treatment of HVOD after HSCT. This study included 19 sHVOD patients. The dosage of defibrotide is 5~60mg/kg per day, intravenous infusion, and the median usage time is 15d (2-61d). The results showed that 42% of patients achieved complete remission (CR), and the overall survival (os) rate 100 days after transplantation reached 32%, and patients could tolerate defibrination well. Since then, multiple clinical trials have confirmed the effectiveness of defibrotide in the treatment of HVOD.

Other foreign studies have provided new evidence that DF protects vascular endothelium after stem cell transplantation. Chemotherapy and transplantation lead to the occurrence of vascular occlusion, and endothelial cell activation caused by occlusion is a common pathological injury. In the experimental design of this study, endothelial cells were incubated with defibrotide in advance to block the activation of p38 and AKT, thereby preventing tissue damage caused by endothelial cell activation. The enrolled cases all met the criteria for severe VOD. Some patients were prone to MOF and even had organ failure. They all showed significant curative effects after treatment with defibrotide. Some VOD patients have achieved complete remission, and their 100-day survival rate has also been significantly improved.
